Are you bringing children under age 18 with you? If so, list names and ages here. If YOU are the parent or legal guardian, there is no need to fill out a separate registration form for your children; but by listing their names here you are assuming responsibility for them while they are at the festival under the strictures of the waiver on the back of this page. If you are NOT the parent or legal guardian, please enclose a signed writ of permission from such, as well as having the child's parent or guardian fill out a separate registration form and sign a waiver.

Dogs: If you need to bring a special needs, please describe the dog and why you need to bring him/her. Please read the rules on dogs! Each dog will be charged $35 admission fee.


Are you vending? Please read, print, and fill out the Vendor Information Form and send it in with your registration form and payment. NOTE: Vendors do not qualify for free or discounted admission, but you also do not have to volunteer for a job when you arrive. Deadline for vendors is MAY 8 - NO EXCEPTIONS!!!

Do you want to hold a class or a ritual? Please fill out the Class and Ritual Volunteer Form and send it in with your registration form and payment. NOTE: Leading a class or ritual counts as part of your community service to the festival, and does not grant you free admission (sorry!). Deadline for classes and rituals is MAY 8 - NO EXCEPTIONS!!!

Pre-payment is STRONGLY ENCOURAGED (it helps me to pay for expenses, postage, printing, site fee deposit, toilet paper, etc.). To receive the current (cheap) price, all registrations must be accompanied by payment. All payments at the gate MUST BE CASH OR MONEY ORDER, and will be the full price of $50, regardless of when you register! NO CHECKS AT THE GATE! Please register by May 15, 2009 (if by mail) or May 20 (if by PayPal), so I have time to send you the Confirmation Flyer and add you to the gate list. To reiterate: If you plan to pay at the gate, you MUST email me your registration info (full name, address, phone, email, medical problems, kids attending, etc.) ahead of time, so I can place you on the gate list. Refunds will be given in full until May 8, at a 50% rate until the Tuesday night before the festival (May 19), and none afterwards. If you send someone else in your place, please let me know beforehand.

Being a voluntary participant at Moondance 2009, at Dragon Hills, Georgia, May 21-25, 2009, I agree to abide by the above rules and regulations. I also understand that if I do not abide by these, I may be escorted from the site without refund. I also agree to hold harmless the owners of the site of Dragon Hills, the organizers and staff of Moondance, and all its representatives, from all claims, actions, expenses and demands in respect to injury, loss or damage to my person or property, however caused, arising in connection with my participation even if the same may have been contributed to or occasioned by the negligence of the owners, the said body, or of any of its representatives.
